At Hearingworks, Kew, we respect your right to privacy. We collect personal information (for example, name and contact details) and health information (for example, medical history relating to your hearing). We use this information in accordance with The Australian Privacy Principles, which are contained in the Privacy Act 1988 and which outline how our organisation must handle, use and manage personal information.
As an Australian hearing services provider, Hearingworks require personal information and health information to provide you with a thorough and quality service. Specifically, we need this information
- For our administrative purposes, including for billing purposes and maintaining contacting with you
- To communicate with third parties, such as those specified under the sub-heading “Disclosure”.
- To inform you of our products and services. Please note that you can opt-out of receiving this information by calling 03 9817 7738.
If you decide not to provide this information, we may not be able to assess or treat you.
Disclosure (sharing of information to third parties)
At Hearingworks, Kew, this information is used to communicate with third parties, in order to facilitate your ongoing care. For example, we may provide your information to:
- Your nominated doctors and/or other medical professionals
- Your nominated teachers and/or other educational professionals such as psychologist’s and speech pathologists
- Government agencies that determine eligibility for hearing services, such as the Office of Hearing Services, Medicare and Work Safe Victoria.
- Our service providers who assist us in providing our goods and services (For example, hearing aid manufacturers when repairing your hearing aids)
- When authorised by law (For example, in an emergency)
We do not disclose your personal information to overseas recipients.
Storage of information
Your information at Hearingworks is stored securely and only Hearingworks’ staff have access to it. Hearingworks takes all reasonable steps to ensure that information collected about you is accurate, complete and up-to-date.
Rights and choices
You may have access to your information on request and if you believe that any of the information is inaccurate you will be able to amend it accordingly. You are under no obligation to provide any information requested by us.
How to make a complaint
If you believe there has been a breach of your privacy, please advise your audiologist. If the matter is not resolved to your satisfaction, you are entitled to contact the Office of the Commissioner for Privacy and Data Protection by phoning 1300 666 444.
